New Patient Forms
As a new patient of Evolve Therapy & Associates, you will need to print out and sign the attached forms and bring them to your first appointment along with your Proof of Insurance and Photo ID. Payment is due at time of services.
If you are attending your sessions via Skype, you will need to download the forms and open them with Adobe Acrobat Reader in order to sign your documents and email them to us. You can download the most current version of Adobe Acrobat Reader for free here.

SKYPE SESSIONS AVAILABLE
If you live to far from our office to come in regularly, we can use Skype to hold sessions from a distance.
Moving or heading away to college? Transitions can be assisted by staying close and personal through Skype.
